Welcome to the 2021 Sports Season!

Please sign up for a time slot if you need a physical examination to participate in the fall sports season.

Arrive 5 minutes before your scheduled time.

Upon arrival at the high school, please call 845-987-3050 extension 12700 to check-in for your appointment. You will be asked health screening questions at this time.

Please wait in your car, and provide a phone number to contact you when we are ready for you to enter the building.

Upon entering the building, you must remain with a mask properly worn at all times while inside and during the exam. Failure to do so will result in you not receiving an examination.

The nurses will direct you once inside the building.

Once your physical examination is complete, please exit the building.
